Forms Auth Using AD

Hi -

I am a systems administrator who is not at all a developer, but I need to implement Forms Authentication on a Windows 2008 R2 server running a third party web application.  So far I haven't been able to find any tutorial that meets my needs.  This basically needs to do the same thing as OWA for Exchange: The user is presented with a form and is authenticated to Active Directory once they enter their credentials.  The only other requirement is that I don't save a user name and password in the form or web.config or anywhere else.  However, if that is a necessity it may be acceptable as long as the account only needs read access to the OU containing the user accounts.

I am looking for something that will show me how to create the authentication form, as well as what needs to be done to direct the user to the form and any IIS-specific settings that may need to exist.  A sample form that I can start with would probably be ideal. 

Thanks for any help with this and hopefully I'm not asking for too much.
